Thank you for your interest in enrolling in modeling classes at Venus Models Agency and Academy.

Please understand that you are not considered registered for classes until this form is completed and sent to Giselle Temple along with your $5,000 tuition payment for 5 classes and graduation show.

Tuition is not refundable and it is the responsibility of the student to make sure they can attend one of the times for all 5 classes. during October/Nov/Dec 2010.
